Integration Readiness Assessment
COPE Health Solutions’ Integration Readiness Assessment will help organizations take the first step in aligning and preparing for funding opportunities and regulatory changes brought about by State and Federal Health Reform. Through the Integration Readiness Assessment, we will:
- Assess the current care transitions and coordination processes and resources across the continuum of care
- Identify best practices currently in place that could be leveraged or coordinated with other providers and resources
- Identify gaps between existing reality and vision for clinical integration as defined by the organization and physicians
- Identify industry best practices and additional resources that should be considered as solutions to bridge the gaps across the continuum of care
